tick: broadcast-hrtimer: Fix a race in bc_set_next

When a cpu requests broadcasting, before starting the tick broadcast
hrtimer, bc_set_next() checks if the timer callback (bc_handler) is active
using hrtimer_try_to_cancel(). But hrtimer_try_to_cancel() does not provide
the required synchronization when the callback is active on other core.
The callback could have already executed tick_handle_oneshot_broadcast()
and could have also returned. But still there is a small time window where
the hrtimer_try_to_cancel() returns -1. In that case bc_set_next() returns
without doing anything, but the next_event of the tick broadcast clock
device is already set to a timeout value.
In the race condition diagram below, CPU #1 is running the timer callback
and CPU #2 is entering idle state and so calls bc_set_next().
In the worst case, the next_event will contain an expiry time, but the
hrtimer will not be started which happens when the racing callback returns
HRTIMER_NORESTART. The hrtimer might never recover if all further requests
from the CPUs to subscribe to tick broadcast have timeout greater than the
next_event of tick broadcast clock device. This leads to cascading of
failures and finally noticed as rcu stall warnings

The comment that hrtimer cannot be armed from within the callback is
wrong. It is fine to start the hrtimer from within the callback. Also it is
safe to start the hrtimer from the enter/exit idle code while the broadcast
handler is active. The enter/exit idle code and the broadcast handler are
synchronized using tick_broadcast_lock. So there is no need for the
existing try to cancel logic. All this can be removed which will eliminate
the race condition as well.

The reason is that the code in collect_expired_timers() uses jiffies
unprotected:
if (next_event > jiffies)
base->clk = jiffies;
As the compiler is allowed to reload the value base->clk can advance
between the check and the store and in the worst case advance farther than
next event. That causes the timer expiry to be delayed until the wheel
pointer wraps around.
Convert the code to use READ_ONCE()

ENOTSUPP is not supposed to be returned to userspace. This was found on an
OpenPower machine, where the RTC does not support set_alarm.
On that system, a clock_nanosleep(CLOCK_REALTIME_ALARM, ...) results in
"524 Unknown error 524"
Replace it with EOPNOTSUPP which results in the expected "95 Operation not
supported" error.

[ Upstream commit 692117c1f7a6770ed41dd8f277cd9fed1dfb16f1 ] Warning when p == NULL and then proceeding and dereferencing p does not make any sense as the kernel will crash with a NULL pointer dereference right away. Bailing out when p == NULL and returning an error code does not cure the underlying problem which caused p to be NULL. Though it might allow to do proper debugging. Same applies to the clock id check in set_process_cpu_timer(). Clean them up and make them return without trying to do further damage. ntp: Limit TAI-UTC offset
[ Upstream commit d897a4ab11dc8a9fda50d2eccc081a96a6385998 ] Don't allow the TAI-UTC offset of the system clock to be set by adjtimex() to a value larger than 100000 seconds. This prevents an overflow in the conversion to int, prevents the CLOCK_TAI clock from getting too far ahead of the CLOCK_REALTIME clock, and it is still large enough to allow leap seconds to be inserted at the maximum rate currently supported by the kernel (once per day) for the next ~270 years, however unlikely it is that someone can survive a catastrophic event which slowed down the rotation of the Earth so much. timekeeping: Repair ktime_get_coarse*() granularity

Jason reported that the coarse ktime based time getters advance only once
per second and not once per tick as advertised.
The code reads only the monotonic base time, which advances once per
second. The nanoseconds are accumulated on every tick in xtime_nsec up to
a second and the regular time getters take this nanoseconds offset into
account, but the ktime_get_coarse*() implementation fails to do so.
Add the accumulated xtime_nsec value to the monotonic base time to get the
proper per tick advancing coarse tinme.

ntp: Allow TAI-UTC offset to be set to zero

The ADJ_TAI adjtimex mode sets the TAI-UTC offset of the system clock.
It is typically set by NTP/PTP implementations and it is automatically
updated by the kernel on leap seconds. The initial value is zero (which
applications may interpret as unknown), but this value cannot be set by
adjtimex. This limitation seems to go back to the original "nanokernel"
implementation by David Mills.
Change the ADJ_TAI check to accept zero as a valid TAI-UTC offset in
order to allow setting it back to the initial value.

timekeeping: Force upper bound for setting CLOCK_REALTIME
[ Upstream commit 7a8e61f8478639072d402a26789055a4a4de8f77 ] Several people reported testing failures after setting CLOCK_REALTIME close to the limits of the kernel internal representation in nanoseconds, i.e. year 2262. The failures are exposed in subsequent operations, i.e. when arming timers or when the advancing CLOCK_MONOTONIC makes the calculation of CLOCK_REALTIME overflow into negative space. Now people start to paper over the underlying problem by clamping calculations to the valid range, but that's just wrong because such workarounds will prevent detection of real issues as well. It is reasonable to force an upper bound for the various methods of setting CLOCK_REALTIME. Year 2262 is the absolute upper bound. Assume a maximum uptime of 30 years which is plenty enough even for esoteric embedded systems. That results in an upper bound of year 2232 for setting the time. Once that limit is reached in reality this limit is only a small part of the problem space. But until then this stops people from trying to paper over the problem at the wrong places. hrtimer: Initialize cpu_base.next_timer to NULL conditionally
hrtimer_cpu_base.next_timer stores the pointer to the next expiring timer in a CPU base. This pointer is used to check whether a hrtimer which is removed is the hrtimer which is the first to expire in the CPU base. To get the estimated sleep length cpuidle_select call from governor depends on hrtimer_next_event_without() which does __hrtimer_get_next_event() excluding tick_sched_timer. cpu_base.next_timer is made NULL unconditionally but actual timer assignment is skipped in case of exclude call. So this pointer stays NULL and we skip programming the hrtimer event even though the hrtimer being removed is the first one to expire. This may result in an unwanted old event getting triggered. Fix this by initializing cpu_base.next_timer to NULL only for non exclude calls. timers, sched/clock: Hook into s2idle freeze path
The sched_clock() suspend/resume callbacks are called from a syscore_ops structure that's registered after the timekeeping syscore_ops. With the suspend-to-idle method of suspend we will freeze the tick and timekeeping subsystem, but we don't suspend the sched clock here, because syscore ops are not called in suspend to idle. Hook this up directly in the timekeeping freeze path when we go idle on the last CPU. This way, the sched clock doesn't jump across a suspend/resume cycle. posix-cpu-timers: Unbreak timer rearming
commit 93ad0fc088c5b4631f796c995bdd27a082ef33a6 upstream.
The recent commit which prevented a division by 0 issue in the alarm timer
code broke posix CPU timers as an unwanted side effect.
The reason is that the common rearm code checks for timer->it_interval
being 0 now. What went unnoticed is that the posix cpu timer setup does not
initialize timer->it_interval as it stores the interval in CPU timer
specific storage. The reason for the separate storage is historical as the
posix CPU timers always had a 64bit nanoseconds representation internally
while timer->it_interval is type ktime_t which used to be a modified
timespec representation on 32bit machines.
Instead of reverting the offending commit and fixing the alarmtimer issue
in the alarmtimer code, store the interval in timer->it_interval at CPU
timer setup time so the common code check works. This also repairs the
existing inconistency of the posix CPU timer code which kept a single shot
timer armed despite of the interval being 0.
The separate storage can be removed in mainline, but that needs to be a
separate commit as the current one has to be backported to stable kernels.

hrtimer: Don't drop the base lock when migration during isolation
The current code drops the base lock and wait for the running hrtimer's expiry event to be processed on the isolated CPU. This leaves a window, where the running hrtimer can migrate to a different CPU or even get freed. The pinned hrtimers that are maintained in a temporarily list also can get freed while the lock is dropped. The only reason for waiting for the running hrtimer is to make sure that this hrtimer is migrated away from the isolated CPU. This is a problem only if this hrtimer gets rearmed from it's callback. As the possibility of this race is very rare, it is better to have this limitation instead of fixing the above mentioned bugs with more intrusive changes. timer: Update code that migrates timers and hrtimers during isolation
__migrate_timers() can be called from both hotplug and isolation contexts. When called from the isolation context, we might sometimes encounter running timers. This is OK since the currently running or just expired timers are off of the timer wheel and so everything else can be migrated off. In the case of hrtimers, we must wait until all callbacks have finished. However, a udelay is important while waiting to allow for store-exclusive fairness when run_hrtimer is attempting to grab the hrtimer base lock. hrtimer: update timer->state with 'pinned' information
'Pinned' information would be required in migrate_hrtimers() now, as we can migrate non-pinned timers away without a hotplug (i.e. with cpuset.quiesce). And so we may need to identify pinned timers now, as we can't migrate them. This patch reuses the timer->state variable for setting this flag as there were enough number of free bits available in this variable. And there is no point increasing size of this struct by adding another field. posix-timers: Fix division by zero bug
commit 0e334db6bb4b1fd1e2d72c1f3d8f004313cd9f94 upstream. The signal delivery path of posix-timers can try to rearm the timer even if the interval is zero. That's handled for the common case (hrtimer) but not for alarm timers. In that case the forwarding function raises a division by zero exception. The handling for hrtimer based posix timers is wrong because it marks the timer as active despite the fact that it is stopped. Move the check from common_hrtimer_rearm() to posixtimer_rearm() to cure both issues.
