Unfortunately "system/accel-ops.h" handlers are not only
system-specific. For example, the cpu_reset_hold() hook
is part of the vCPU creation, after it is realized.
Mechanical rename to drop 'system' using:
$ sed -i -e s_system/accel-ops.h_accel/accel-cpu-ops.h_g \
$(git grep -l system/accel-ops.h)

The heavily imported "system/cpus.h" header includes "accel-ops.h"
to get AccelOpsClass type declaration. Reduce headers pressure by
forward declaring it in "qemu/typedefs.h", where we already
declare the AccelCPUState type.
Reduce "system/cpus.h" inclusions by only including
"system/accel-ops.h" when necessary.

This started as a clean-up to properly pass a Error handler to the
gdbserver_start so we could do the right thing for command line and
HMP invocations.
Now that we have cleaned up foreach_device_config_or_exit() in earlier
patches we can further simplify by it by passing &error_fatal instead
of checking the return value. Having a return value is still useful
for HMP though so tweak the return to use a simple bool instead.

Headers in include/sysemu/ are not only related to system
*emulation*, they are also used by virtualization. Rename
as system/ which is clearer.
Files renamed manually then mechanical change using sed tool.

Move GdbCmdParseEntry and its associated types into a separate header
file to allow the use of GdbCmdParseEntry and other gdbstub command
functions outside of gdbstub.c.
Since GdbCmdParseEntry and get_param are now public, kdoc
GdbCmdParseEntry and rename get_param to gdb_get_cmd_param.
This commit also makes gdb_put_packet public since is used in gdbstub
command handling.
