QAPI's 'prefix' feature can make the connection between enumeration
type and its constants less than obvious. It's best used with
restraint.
QCryptoHashAlgorithm has a 'prefix' that overrides the generated
enumeration constants' prefix to QCRYPTO_HASH_ALG.
We could simply drop 'prefix', but then the prefix becomes
QCRYPTO_HASH_ALGORITHM, which is rather long.
We could additionally rename the type to QCryptoHashAlg, but I think
the abbreviation "alg" is less than clear.
Rename the type to QCryptoHashAlgo instead. The prefix becomes to
QCRYPTO_HASH_ALGO.

Almost all functions that access bs->file already take the graph
lock now. Add locking to the remaining users and finally annotate the
struct field itself as protected by the graph lock.

qemu_uuid_unparse() includes a trailing NUL when writing the uuid
string and the buffer size should be UUID_FMT_LEN + 1 bytes. Add a
define for this size and use it where required.

We have two inclusion loops:

I believe these go back to Emanuele's reorganization of the block API,
merged a few months ago in commit d7e2fe4aac.
Fortunately, breaking them is merely a matter of deleting unnecessary
includes from headers, and adding them back in places where they are
now missing.
