Eduardo Habkost 3e2c0e062f cpu: Eliminate cpudef_init(), cpudef_setup() ...

x86_cpudef_init() doesn't do anything anymore, cpudef_init(),
cpudef_setup(), and x86_cpudef_init() can be finally removed.
