xiaoqiang zhao db0f888848 ioapic: QOM'ify ioapic ...

Convert 'init' function to QOM's 'realize' for ioapic and kvm-ioapic.
Change variable 'ioapic_no' from static to global. Then we can drop
the 'instance_no' function argument.
